Records Office Forms

The Records Office offers several services that require a form submission. The forms are listed below.

There are three methods to submit non-electronic forms to our office. A photo identification is required with all form submissions that request updates to name, address, phone numbers, and/or email addresses. Please review each form for any additional documentation that may be requested.

  1. In person: You may submit records forms to our office in Medlin Campus Center,
    Suite 3500 in Jamestown. We are open 8 a.m.-6 p.m. Monday-Thursday and 9 a.m.-5 p.m. Friday.
  2. eFax: You may fax us the form with the appropriate documentation to 336-458-2359.
  3. Scan and email: Please scan and email the form and appropriate documentation to [email protected]
