Faculty Testing Instruction Form [PDF]
This is the only accepted version of the Faculty Testing Instruction Form that will be accepted.
- It can be used for DAS and non DAS students.
- Must be submitted 48 hours before the test’s start date and time. This time restriction helps to ensure DAS students testing on the Jamestown Campus have adequate time to make their required appointment. DAS students are required to schedule their testing appointment at least 24 hours in advance due to the limited availability of distraction reduced rooms. The times are necessary as we want to ensure every student receives the accommodations that they are allowed.
- Do not include extended time calculations on the Testing Instruction form as our software automatically matches students and their accommodations.
- Do not use phrasing of “unlimited” or “not timed”. For time calculations enter the actual time that is allowed to students taking the test in your classroom.
- The Assessment Center can accommodate any student who is unable to test in the classroom because of an accident, illness or other excused absence from class. Students who are contagious are not to test in the Assessment Center until they are no longer contagious (for example, chicken pox or strep throat).
- The Assessment Center cannot accommodate entire classes, including final exams unless the class is an alternate delivery course (i.e. distance learning or the web enhanced). Seating within the Assessment Center is limited and during busy times, students may have to wait for an available desk or computer station.
The Assessment Center is unable to accommodate out of class assignments tests.
The following instructions will aid the student with scheduling a test using accommodations. FYI – DAS personnel are also able to assist with scheduling of test appointments.
Go to www.registerblast.com/gtcc/exam.
- Choose Jamestown Campus.
- Scroll down the list to find Testing with Accommodations and select one of the following options.
- Testing with Accommodations (Scribe, Wheelchair, etc.)
- Testing with Accommodations in Open Area
- Testing with Accommodations in Separate Room (Distraction reduced environment)
- Read the Exam Description.
- Confirm your accommodations. (If you believe them to be in error, please contact DAS before coming to the Assessment Center.)
- Select a date on which you want to test.
- Select the time to start your test.
- Enter your name and email address information.
- Agree to the guidelines.
- Enter the requested information
- Click the green ADD TO CART button.
- Review and click the COMPLETE REGISTRATION.
You should see an immediate confirmation of your successful appointment. You will receive a confirmation via email.
You will also receive an email reminder 24 hours before your scheduled test time.

The most important thing for your students to know is the info you have included on the testing instruction form. That information guides what Assessment Center staff will permit.
- Assessment Center without a valid photo id will not be allowed to test.
- Please inform your students that they will need to be able to furnish Assessment Center staff with the following information: Instructor’s name; the course number; if it is a distance learning course; and the test name or number or the chapters that the test covers. This is necessary as some instructors teach 2-3 different courses and we want to ensure the student is provided with the correct test.
- Please ensure students are aware of materials that they may access during the test. For example, if the faculty instruction form identifies that a scantron must be used, it is the student’s responsibility to provide it.
- Here are some examples:
- If the use of a calculator is noted then the student is responsible to ensuring the calculator they bring with them is of the correct type. If a basic calculator is allowed and the student brings a graphing calculator the student will be unable to use the calculator.
- If a notation is made on the form that two (2) 3"x5" index cards can be used then a sheet of paper filled with notes is not appropriate and will not be allowed.
Staff in the Assessment Center will not accept any materials from students that are not directly related to the test the student is taking.
Students need to have the following with them when they arrive at the Assessment Center
- A valid photo ID. (A copy of ID on student’s phone is not acceptable.)
- Students should know the name of the instructor for whom they are testing.
- Students should know the name of the student’s course for which they are testing.
- If the test is online students should have their login information written down, not on their phone, when they arrive at the Assessment Center.
Students should be aware if they need to bring any of the following with them to the Assessment Center.
- Scantron (The Assessment Center does not furnish this item.)
- Calculator (The Assessment Center does not have calculators to loan out.) If a calculator is allowed, please ensure you bring the one specified by your instructor,
- Notes (If yes, know the specifics; half sheet of paper, three 4x6 cards, etc.)
